2012/2013 MRFC Registration & Forms

Thank you for visiting the MRFC Player Registration and Forms page.  Please visit the tabs on the left to find all player registration forms, tryout form, scholarship requests, game day forms, the US Club Soccer Staff Background Check Link and more!!

As a reminder if you want to make monthly payments you will need to login each month and pay with your credit card, demosphere (this registration system) does NOT have an auto-pay program, you must manually do it yourself.  Thanks.

All players need to submit the following 5 items at registration

(forms found in the tabs on the left):

1) MRFC Parent/Player Code of Conduct Form

2) MRFC Player Photo Release Form

3) US Club Soccer Youth Player Registration and Medical Treatment Authorization Form

4) PSPL Concussion Compliance Form

5) A copy of your players birth certificate (for all new AND returning players)
