MRFC - FORMS
Premier Registration Forms:
- MRFC Tryout Registration Form - Use this form to pre-register for MRFC 2011 Tryouts.
- 2011-2012 MRFC WYS Age Verification Chart - This is to help you verify the correct age group your child can play.
- WYS Medical Release Form - To be completed by parent/guardian, must be turned in to team manager.
- Code of Conduct Form - To be completed by parent/guardian & player, turn in to team manager.
- Photo Release Form - To be completed by parent/guardian and turned in to team manager.
- WYS Birth Date Verification Form - To be completed by parent/guardian (only if your player is new to MRFC for the 2011/2012 season). Turn in to team manager.
- PSPL Player Registration Medical Authorization Form - Required for all players competing in PSPL, to be completed by parent/guardian, turn in to team manager.
- PSPL Concussion Form - complete this for teams registered in the PSPL
Premier Payment Options:
- 2011-2012 Premier Payment Plan Form - This form must completed and turned in to team manager. Team manager will submit along with team registration worksheet to MRFC Treasurer.
- Premier Scholarship Request
Premier Managers Forms:
- Team Registration Worksheet - To be completed by team manager and turned in to the MRFC Registrar. It is the team managers responsibility to make sure that all the information is completed on the worksheet prior to turning in to the MRFC Registrar.
- Premier Girls Uniform Order Form - Team Managers please complete.
- Premier Boys Uniform Order Form - Team Managers please complete.

Coaches / DOC Forms:
- Coaches Application Form #900 - Complete and mail to the address on form.
- Coaches Contract Form #901 - To be completed by the DOC and signed by the Head Coach upon agreement.
- Coaches Code of Conduct Form #902 - To be completed by the coach and returned to MRFC VP of Competition annual
- Coaches Evaluation Form #903 - To be completed by the DOC for every Head Coach at least once per season.
- Player Evaluation Form #904 - To be completed by coaches for every player at least once per season.
