MRFC SOCCER FORMS

• Coaches Application Form - Complete and mail to the address on form.
  
• Code of Conduct Form - To be completed by parent/guardian & player, turn in to team manager.
  
• Medical Release Form - To be completed by parent/guardian, must be notarized and turned in to team manager.
  
• Payment Plan - This form must completed and turned in to team manager.  Team manager will submit along with team registration worksheet to MRFC Treasurer.
• Photo Release Form - To be completed by parent/guardian and turned in to team manager. 
  
• Player Registration Medical Authorization Form - Required for all players competing in PSPL, to be completed by parent/guardian, turn in to team manager.
• Scholarship Request - This form can be mailed to MRFC or turned in to team manager who will submit with team registration worksheet.
  
• Team Registration Worksheet - To be completed by team manager and turned in to the MRFC Treasurer.  It is the team managers responsibility to make sure that all the information is completed on the worksheet prior to turning in to the MRFC Treasurer.

• Volunteer Discount Request - To be completed and turn in to MRFC Treasurer. 
  
• Washington Youth Soccer Birth Date Verification Form - To be completed by parent/guardian (only if your player is new to MRFC for the 2010/2011 season).  Turn in to team manager.
